Hi. I need to be reimbursed on my doctor's visit and the prescribed medicines. How do I get reimbursed?

In order to file a claim on your insurance plan you would need to log into the MyAccount section of the website with your certificate number and date of birth and fill out a claim form. You can find the claim forms under the fulfillment documents section of the MyAccount portal. Follow the submission instructions on the form.
